Could you all state us the situations (if any) where you had to burn a player's hand

In fact i mean you as a floor are called for a dispute and you have to declare a player's hand is dead

there are many situations that could warrant the floor to declare a player's hand dead. Too many hole cards...holding two identical cards...holding cards without calling a bet from the last street...declaring a fold and refusing to surrender the hand. I imagine there are probably too many to list.
